Along with her parents, Jeanette is preceded in death by her husband, Joe Formby, one daughter, Cindi Johnson, one son, Floyd Richard Gibbs, grandsons, Jacob Formby and Brian Lindsay, her siblings, Theodore "Cotton" Goolsby, Walter "Buddy" Goolsby, Lois Ella Goolsby Birk.

Survivors include, one son, Lamar Formby, seven grandchildren, Mark Johnson, Laura Johnson, Joshua Formby, Job Formby, Grace Elizabeth, Julia Zachary, Jocelyn Marie, five great grandchildren, Adeline Brown Johnson, Micah Asher Formby, Jonah Mavyn Formby, Ella Ezme Formby, Jace Havox Formby. She also had two loving nieces that thought of her as their second mother, Beverly Berry of Inverness, FL and Brenda DeCaprio of Ellenboro, NC and many other nieces and nephews.

Funeral Services will be held on Wednesday, January 19, 2022 at Blue Lake Baptist Church. Visitation will begin at 10:00 A.M. with Funeral Service to follow at 11:00 A.M. Interment will follow at the Blue Lake Baptist Cemetery.
